INFORMED CONSENT FOR DIETITIAN SERVICES

 

Nature and purpose of the Dietitian Service

As part of providing a dietetic service including assessments, dietary interventions and recommendations to you, Marnie Buller, will need to collect and record personal information about you. This information is a necessary part of the dietary assessment and treatment.

Dietitian consultations vary based on individual needs and the specific health concerns or goals you bring forward. There are various approaches I may use to assess your nutritional needs and make recommendations to improve your overall health through diet and lifestyle changes. It’s important to note that your active participation is key to achieving the best results. To maximise the success of our work together, you will need to apply the dietary strategies we discuss during our sessions, as well as in your daily life.

 

Our first session will focus on evaluating your current health status, eating habits, and nutritional needs. By the end of this assessment, I will provide initial recommendations, and we may set some personalised strategies for optimal nutrition.  Subsequent reviews will build on the initial plan and make necessary tweaks for ongoing success.  If you have any questions about the process, we can address them at any point. You are free to discontinue consultations at any time, and if at any point you feel uncomfortable during a session, you can request to pause or end it.

Access to your information

As part of providing dietitian consultations, relevant personal and health information will be collected and recorded to ensure the most appropriate and thorough service. This information helps me assess your nutritional needs and develop suitable dietary recommendations. All details will be securely stored. I am required to keep your records for a minimum of 7 years, or if you are under 18 at the time of treatment, until you reach 25 years of age. You may request access to your file at any time, subject to the exceptions outlined in the National Privacy Principles.

Confidentiality and Privacy

All personal information gathered by the dietitian during the provision of dietetic services will remain confidential and secure except when:

1.      it is subpoenaed by a court, or disclosure is required or authorised by law; or

2.      failure to disclose the information would place you or another person at risk of harm; or

3.      given your prior approval, or consent of a parent or guardian who is legally authorised to act on your behalf, to provide a written report to another professional or agency, or discuss information with another person e.g., parent or employer; or

4.      you would reasonably expect your personal information to be disclosed to another professional or agency and disclosure is directly related to the primary purpose for which it was collected such as to inform your GP of treatment and progress; or

5.      clinical consultation with another professional is required to provide better clinical services (identifying details will remain confidential).

If during the course of your work with Marnie Buller, she becomes aware of risk to someone’s life, health, or wellbeing, she is required by policy of the state and federal governments to report the matter to the appropriate agencies.

Session Fees 

My agreed fee is $165 inc. GST per initial consultation or extended review session (50-minute duration) or $121 inc. GST per review consultation (35-minute duration) and is payable at the time of consultation.

Private Health Insurance

Some health insurance policies cover dietetic services depending on the policy. Please check with your insurance company as to what cover you are eligible.

Medicare Rebates

Under the Medicare Benefits Schedule (Better Access), Medicare rebates are available to clients with an assessed chronic disease or eating disorder. Medicare rebates are accessed with a Chronic Disease Management Plan (CDMP) or Eating Disorder Treatment and Management Plan (EDP), and referral from a medical practitioner.  The rebate is limited to the allocated number of sessions on the CDMP or EDP. Please note the maximum number of sessions per CDMP referral is 5 and EDP is 20 and rebates will not be given beyond the number stated on the referral.

Cancellation Policy

If for some reason you need to cancel or postpone your appointment, please give at least 24 business hours’ notice – as a $50 fee will be charged for all no-shows or late cancellations to an appointment. Emergency

In case of emergency, please go to the emergency department of the nearest hospital. For further information refer to the website for your local health district.

Consent

By booking an appointment, you agree you have read and understood the above consent form provided by Marnie Buller and have had the opportunity to have any questions answered. You also understand and agree to pay all associated session fees.
